Acute Care Visit This visit is for the evaluation and treatment of sudden onset or worsening health issues. Examples of conditions treated include: Fever, chills, or flu-like symptoms Minor injuries (e.g., sprains, strains, cuts) Headaches, dizziness, or unexplained pain Abdominal pain, nausea, or vomiting Rashes, allergic reactions, or insect bites Urinary symptoms, such as pain or burning Our healthcare provider will perform an examination, discuss your symptoms, and determine appropriate tests, treatments, or medications. If needed, the provider will recommend follow-up care or referrals to specialists. Book now 03. Prescription Renewals This appointment is intended for patients who need a short-term or time-sensitive renewal of a current medication while they await an appointment with their primary care provider (PCP) to ensure there is no interruption in treatment. 1-2 Medications / $30 / 15 minutes 3-4 Medications / $50 / 25 minutes 5+ Medications / $70 / 35 minutes Conditions addressed in this appointment include: Renewal of medications for chronic conditions that require a temporary refill Medications for mental health conditions to prevent a lapse in treatment Other time-sensitive renewals of medications Our healthcare provider will review your medication history, confirm the need for a short-term renewal, and provide a temporary prescription if appropriate. Please note: This visit is not intended for new prescriptions, medication adjustments, or comprehensive evaluations. Patients are advised to schedule these appointments with their primary care provider. Book now 05. Forms and Paperwork This appointment is for patients who need assistance with time-sensitive notes, forms and paperwork. Services provided include: Sick notes for work, school, daycare ($30) Notes requested by insurers for massage, orthotics, physiotherapy, etc. ($30) Completion of sickness benefits form for unemployment insurance ($40) School/College/University Medical Certificate ($40) WSIB paperwork related to workplace injuries or illnesses (No cost - billed to WSIB) Medication Administration form ($30) Driver's Medical (form + physical assessment - $150) Pre-employment/Employment Medical (form + physical assessment - $150) Disability Tax Credit Paperwork - Due to strict eligibility criteria set by the CRA, this paperwork requires a two-step process. Step 1: Medical Record Review ($50) - you must provide copies of your relevant medical records for review. If the provider feels your documentation adequately supports a DTC application, you will be booked for Step 2. Please note that this fee does not guarantee the forms will be completed. Step 2: Form Completion ($100) Other paperwork (cost is at the discretion of the provider) Please note: This appointment is not intended for medical consultations or evaluations. Our healthcare provider will review the required documents and provide the necessary signatures, medical information, or details to support your application. If further medical evaluation is needed, you may need to schedule a separate appointment. book now 02. procedure visit This visit is designed for straightforward procedures that require timely access to care but do not require a full assessment or acute care visit. Examples include: Suture or staple inspection/removal Minor wound checks Dressing changes Ear cleaning/irrigation Wart treatment (If assessment and confirmation of diagnosis is required, please book acute care visit - this appointment is for liquid nitrogen treatment only/follow up treatments once wart is confirmed) Pregnancy confirmation IUD string check During this appointment, our healthcare provider will complete the procedure only. If you are experiencing new symptoms, complications, or require a full medical assessment, please book an acute care visit instead. Book now 04. acute care mental health visit (Virtual) This appointment is scheduled with our Registered Social Worker (RSW) and may be covered by your extended health benefits plan. This appointment is for individuals of any age experiencing urgent mental health concerns including but not limited to: Recent increase in anxiety, panic attacks Worsening of mental health conditions Recent traumatic event Self harm concerns Domestic violence or abuse disclosures Emotional distress Other emotional distress Our provider will offer a timely assessment, supportive care, short term coping strategies and treatment recommendations. If your regular therapist isn't available, you need occasional mental health support, or want help finding ongoing resources, this visit is for you. This visit is not intended for long-term counseling but rather for immediate support and intervention. NOTE: At this time, this appointment is available by phone or video call only. book now 06. Follow up This appointment is only for patients who: a) do not have a primary care provider (PCP) and; b) require a follow-up appointment after being seen by our provider for an acute concern. NOTE: Follow-up appointments cannot be booked online. If a follow-up is recommended during your initial visit, your provider will schedule it for you. If no follow-up was arranged but you feel one is needed, please contact the clinic to inquire about booking. Follow up appointments are intended to help ensure continuity of care for patients without an established PCP until they can establish care with one. Services provided during this visit may include: Follow-up on recent medical treatments Ongoing management of chronic conditions (temporary care) Monitoring of recovery after illness or injury Prescription refills, if needed, within the scope of follow-up care Please note: If you have a PCP, you should schedule follow-up care directly with them. This visit is specifically for those without a PCP. Contact to book What we don't offer Emergency Care (e.g. chest pain, stroke symptoms, difficulties breathing - call 911) Ongoing management of chronic conditions (e.g. diabetes, high blood pressure, high cholesterol - if you require chronic disease management and do not have a family doctor, you can get care at the St. Thomas - Elgin Community Health Hub or at Hint of Health Inc.) Completion of Ontario Disability Support Program paperwork - these forms require detailed, ongoing documentation of medical history, treatments and functional impairments over time and our clinic does not have the medical records necessary to accurately and appropriately complete these types of assessments. Prescription of controlled substances (e.g. pain medication, stimulants, benzodiazepines - The prescribing and monitoring of these medications require a comprehensive, ongoing therapeutic relationship which cannot be guaranteed in an acute care setting.) Immunizations (e.g. influenza, COVID-19, scheduled childhood immunizations - if you require an immunization, please consider seeing a pharmacist or visiting Hint of Health if you do not have a family physician.)

Frequently Asked Questions Why do I Have to Pay Out of Pocket for Your Services? In Ontario, most healthcare services are funded through OHIP (Ontario Health Insurance Plan). Unfortunately, independent Nurse Practitioner (NP) clinics do not receive direct OHIP funding for patient visits. As a result, NPs operating in private practice must charge out-of-pocket fees for services, similar to other healthcare providers such as physiotherapists, chiropractors, and registered massage therapists. What’s Included in the Fee? For each visit, the fee covers specific care and/or treatment provided at the visit. This could include things like a medical history, physical assessment and treatment plans including medication prescriptions or other services. What About Publicly Funded Health Care? While we fully support the publicly funded health care system in Ontario, we recognize that there are situations where additional services or alternatives may be necessary. Our fee-for-service model allows us to provide these services in a way that complements the public system and meets your individual needs. What is your payment process? Payment for services is due at the time of your visit. We accept various payment methods including credit card, cash, VISA debit or e-transfer. Do my health benefits cover your services? Your health benefits may cover some or all of the costs associated with care provided by a Nurse Practitioner. We can provide you with the necessary information and documentation to submit to your insurance company for reimbursement. What if I have questions about your fees? We strive to be transparent about our pricing. If you have any questions about the cost of a specific service, please feel free to ask.     About us Ontario’s healthcare system is currently facing significant challenges, and our community is no exception. Our emergency department is overwhelmed by persons seeking care for acute but non-life threatening conditions, leading to excessively long wait times and overcrowding. There are primary care physician shortages leaving many residents without access to a family doctor and consistent care. Not only does this compel someone to use the emergency room or walk-in clinic more often, but they often seek care at later stages, resulting in more advanced health conditions. NPACC seeks to fill a critical healthcare gap in our community by providing residents with timely, accessible care for non-life-threatening but urgent medical issues, thus reducing the burden on emergency departments and filling gaps in primary care access. More specifically, NPACC hopes to reduce emergency room overcrowding and wait times by diverting patients with urgent needs, allowing the hospital to focus more on critical emergencies. In regards to primary care, NPACC wants to ensure that residents receive access to timely medical attention for acute issues, despite not having a family physician. We would further support our communities primary care sector and family physicians by easing the strain and pressure placed on them due to the high demand of their services and thus reduce the backlog they are facing, causing patients to wait many weeks for their health care concerns to be addressed. Lastly, as mentioned above, by providing patients with timely access to care for acute health concerns, NPACC will prevent minor issues from escalating into severe conditions requiring hospitalization which will improve health outcomes and further decrease the burden on our overwhelmed health care system.
