This visit is for infants and toddlers experiencing non-emergency health concerns that need prompt attention. Our nurse practitioner will assess, diagnose, and treat common pediatric illnesses and minor injuries in a child-friendly environment. We aim to provide timely, compassionate care to help your child feel better quickly.

Adolescent Acute Care Visit (13-17)

This visit is for evaluation and treatment of adolescents experiencing a sudden onset or worsening health issues.

Our Nurse Practitioner will perform an examination, discuss your symptoms, and determine appropriate tests, treatments, or medications. If needed, the provider will recommend follow-up care or referrals to specialists.

Prescription Renewal (1-2 Medication)

$30

This appointment is intended for patients who need a short-term or time-sensitive renewal of 1-2 current medication while they await an appointment with their primary care provider (PCP) to ensure there is no interruption in treatment.

Please note: This visit is not intended for new prescriptions, medication adjustments, or comprehensive evaluations. You are advised to schedule these appointments with their primary care provider.

Follow-Up

This appointment includes a consultation to request bloodwork based on your health needs, another providers recommendation or for schooling/work. It also includes a full review and interpretation of your results once available, with results shared securely and clearly explained. Follow-up recommendations will be provided as needed.

This appointment is for time sensitive letters requested by insurers for coverage of massage, orthotics, physiotherapy, etc.

Disability Tax Credit - Medical Records Review (Part 1)

This 30 minute appointment is for medical record review. You must provide copies of your relevant medical records that support your disability. During this review, we assess whether the documentation adequately supports a potential DTC application. The fee covers the review only and does not guarantee that the form will be completed. If it is deemed appropriate to complete the DTC application, a second appointment will be booked for form completion.

Medication Administration Form

This appointment is for the completion of time sensitive medication administration forms required by school or daycare.

School Medical Certificate

This appointment is for students who require a time sensitive medical certificate for school-related purposes such as exemptions from physical activity, or return-to-school clearance.

Sickness Benefits

This appointment is for patients who need assistance with time-sensitive completion of sickness benefits form for unemployment insurance.

Compassionate Care/Family Caregiver Benefits

This appointment is for patients who need assistance with time-sensitive completion of the Medical Certificate form for Compassionate Care Benefits.

Non WSIB Functional Abilities Form

This appointment is for individuals with an injury or illness requiring modifications at work and completion of associated workplace functional abilities form.

Driver's Medical

This appointment is for individuals requiring a time sensitive medical examination to meet licensing requirements for commercial or personal driving as required by the Ministry of Transportation. Please bring any required forms and a list of current medications.

Fee includes appointment cost and paperwork completion.

Disability Tax Credit - Form Completion (Part 2)

If the documentation in Step 1 was sufficient and criteria appear to be met, then this appointment is booked for completion of the DTC form.

Pre-Employment/Employment Medical

This appointment is for individuals requiring a time sensitive medical examination as part of employment onboarding or as required by the employer. Please bring any relevant forms and a list of current medications.

Fee includes appointment cost and paperwork completion.

Sick Note

This appointment is for patients who need assistance with time-sensitive sick notes for work, school or daycare.

WSIB Assessment

This appointment is for individuals with a work-related injury or illness requiring time sensitive assessment or follow-up under WSIB. Please bring your claim number, any relevant paperwork, and a list of current medications.

NOTE: There is no cost to this appointment as it is billed directly to WSIB

Sports Medical Clearance

$140

This appointment includes a medical clearance examination and form completion required for participation in organized sports, school teams, athletic clubs, or recreational leagues.

What’s included: Review of medical history, physical examination, vitals, vision screening (if required) and completion of forms.

Generally intended for healthy youth and adults requiring clearance for non-professional sports. Common for school athletics, boxing, martial arts, dance, or competitive leagues.

Note: Please bring your form(s) to the appointment.

Fee: $140 (includes both the physical exam and form completion)

Procedure visit

Ear Wax Removal

$40

This appointment is for the safe removal of ear wax in one or both ears, using gentle flushing or other appropriate methods. A healthcare provider will assess the ears and perform the procedure if clinically indicated.

Wound Assessment/Wound Care

$40

This appointment is for the follow-up evaluation and care of an existing wound. It includes monitoring healing progress, checking for infection, and updating wound care as needed.

Note: This visit is not intended for new or acute injuries. For recent wounds, please book an Acute Care appointment.

IUD String Check

$40

This appointment is for a quick assessment to confirm the presence and position of your IUD strings. A healthcare provider will perform a brief pelvic exam to ensure the IUD remains in place.

Note: This visit is for routine string checks only. If you are experiencing pain, abnormal bleeding, or symptoms of concern, please book an Acute Care appointment instead.

Injection

$30

This appointment is for the administration of a pre-prescribed injection, such as Vitamin B12, hormonal therapies, or other intramuscular or subcutaneous medications. A brief assessment will be completed to ensure it is safe to proceed, followed by the injection.

Note: This appointment does not include a full consultation. If you have new symptoms or need to discuss changes to your treatment, please book an acute care visit.

Suture or Staple Removal

$40

Book this appointment for the safe and sterile removal of surgical staples or sutures. A healthcare provider will assess the healing site and ensure proper wound care during the visit.

Pregnancy Confirmation

$30

This appointment is for confirming pregnancy through beta hCG blood testing after a positive home pregnancy test or suspected pregnancy. A healthcare provider will order the appropriate lab work and follow up with results and next steps based on your individual needs.

Note: This appointment is for confirmation of pregnancy only. If you are experiencing symptoms such as pain or bleeding, please book an Acute Care visit instead.

IUD Removal

$70

This appointment is for the safe removal of an intrauterine device (IUD) by a healthcare provider. The procedure is typically quick and performed in-clinic.

Note: This visit is for routine IUD removals. If you're experiencing pain, abnormal bleeding, or are unsure if removal is appropriate at this time, please book an Acute Care appointment instead.

Skin Assessment and Biopsy

$150

This 45-minute appointment includes a comprehensive skin assessment, skin history review, and risk evaluation for skin cancer or other dermatological concerns ($100). If any suspicious lesions are identified, biopsies may be performed during the same visit.

Each biopsy is an additional $50 per lesion and includes sample collection, submission to pathology and review of results.

NOTE: The appointment price reflects the cost of the skin assessment and biopsy of a single lesion.
